This three-part educational series supports primary care clinicians participating in the CMS GUIDE Model and others serving adults with IDD, including Down syndrome. The series provides a practical, step-by-step framework spanning early identification, diagnostic evaluation and medical management, and post-diagnosis care planning.

CMS GUIDE Model Navigator Course
Navigating Dementia Care in Intellectual Disabilities for CMS GUIDE navigators and related personnel.
Online, On-Demand. 3 - Module Course
Supported by a grant from Special Olympics, this course is now available. It was designed to help navigators meet the CMS GUIDE program’s supplemental education requirements while building expertise in an often-overlooked area of care -- Medicare beneficiaries with intellectual disability and dementia and their caregivers. Delivered in three flexible, on-demand units, the course explores the fundamentals of aging and dementia in intellectual disability, the dynamics of specialized family and caregiver support, and strategies for connecting beneficiaries to community resources for person-centered dementia care planning. Especially valuable for navigators onboarding Medicare beneficiaries with a range of intellectual and developmental disabilities, the course is available anytime through June 2026. A Certificate of Completion will be issued upon completion of the course.
This course is available at no cost for CMS Guide Participant staff, GUIDE Navigators, and personnel of Partner organizations working directly with a GUIDE Participant.
